The Rehab to Home program at Montezuma Nursing & Rehab Center includes the services of trained professionals that are needed for a limited period of time after an injury or illness.

A physical therapist helping to correct strength and balance problems that have made it difficult for a patient to walk or get on and off the bed, toilet or furniture.

Speech therapists helping a person regain the ability to communicate after a stroke.

Occupational therapists helping a person relearn independent self-care in areas such as dressing, grooming and eating.
Montezuma Nursing and Rehab Center provides quality medical, skilled nursing and rehabilitative care through registered and licensed practical nurses, certified nursing assistants, and physical occupational and speech therapists.
This care is complemented by social work, activities, nutrition services and housekeeping and laundry services. The staff works with residents and their families to design an individualized plan of care that best addresses resident needs or those of their loved one.
The facility offers post-acute services for those transitioning from hospital to home. This outcome-focused specialized care helps shorten the hospital stay and lays the groundwork for a successful discharge to home or to a lower level of care.
Montezuma Nursing and Rehab Center's outcome-oriented rehabilitation program offers residents the ability to meet the unique challenges for their rehabilitation in a home-like setting. Functional improvement and a better quality of life for each resident are the goals of the facility's rehabilitation department.
Montezuma Nursing and Rehab Center's team of physical, occupational and speech therapists work together daily to assist and improve mobility, self-care and communications skills for residents who have had strokes, debilitating illnesses, surgeries and other events.
The facility's team develops an individualized plan for each resident to help them reach their maximum physical potential. Therapists work to accommodate each patient's needs and schedule, bringing to rehabilitative care a comprehensive approach and a number of specialized treatment programs.
